If you want to request the use of our facilities for a group/event at Fair Oaks Church, please fill out the Facilities Request Form below. All requests will require approval from the ministry head overseeing the group/event before they will be submitted to our event coordinator for scheduling. Please be patient and flexible if rooms/services are not available

If you'd like to request a memorial service at Fair Oaks Church, please fill out the Memorial Request Form below. Someone from our team will reach out to you shortly with more information and will walk alongside you through this process. We will do the best we can to accommodate requests given what rooms and resource we have available. 

If you'd like to request the promotion of any event or group, please fill out the Publicity Request Form below and give our team at least a 2 week lead time to create necessary promotion graphics and materials. This also gives us enough time to publicize the event as needed. Thank you for your request.
